ALL EVENT MATERIAL (PICTURES, FLYERS, DOCUMENTS, EVALUATIONS_ MUST BE SUBMITTED TO THE MEDIA DEPARTMENT PRIOR TO THE START DATE OF THE EVENT.
USE THIS CHECKLIST TO HELP KEEP TRACK OF WHERE YOU ARE IN THE FORM PROCESS.
THIS PROCESS SHOULD TAKE NO MORE THAN 3 - 4 MONTHS OF PLANNING INCLUDING FORM SUBMISSIONS AND LEADERSHIP APPROVALS.
*Each Form listed below is in the order in which they are to be submitted.
BUSINESS FORMS
ALL EVENTS, MEETINGS OR SERVICES MUST BE APPROVED PRIOR TO ALL FORM SUBMISSIONS

ANNOUNCEMENTS FORM
All Announcements & Media should be submitted four (4) weeks before the start date of your event.
Announcements will run for three (3) consecutive Sundays.
Prior to your event START DATE.
Considerations will be made for special events as determined by Bishop Maynard.

COPY REQUEST FORM
Complete this form to receive copies of your event flyer. Please be sure to contact the church office for all copies at (615) 876-8740.

EVENTS FACILITIES FORM
Submit Form at least four (3) weeks prior to your event start date.
Please allow at least 2 week turnaround time from leadership.
*Please note that private events may have associated fees.

MAINTENANCE REQUEST
Submit this form if any damages, spills or issues that need attention to or repairs at the church.

MINISTRY INFORMATION
Complete this form to keep your ministry information up to date on media platforms.

PROMOTIONAL REQUEST
Form must be submitted in full detail 30 days prior to your event start date
Please Allow at least 1 week for review and preparation.
