BUSINESS FORMS CHECKLIST
ALL EVENT MATERIAL (PICTURES, FLYERS, DOCUMENTS, EVALUATIONS_ MUST BE SUBMITTED TO THE MEDIA DEPARTMENT PRIOR TO THE START DATE OF THE EVENT.
USE THIS CHECKLIST TO HELP KEEP TRACK OF WHERE YOU ARE IN THE FORM PROCESS.
THIS PROCESS SHOULD TAKE NO MORE THAN 3 - 4 MONTHS OF PLANNING INCLUDING FORM SUBMISSIONS AND LEADERSHIP APPROVALS.
*Each Form listed below is in the order in which they are to be submitted.
ALL EVENTS, MEETINGS OR SERVICES MUST BE APPROVED PRIOR TO ALL FORM SUBMISSIONS
All Announcements & Media should be submitted four (4) weeks before the start date of your event.
Announcements will run for three (3) consecutive Sundays.
Prior to your event START DATE.
Considerations will be made for special events as determined by Bishop Maynard.

Submit Form at least four (3) weeks prior to your event start date.
Please allow at least 2 week turnaround time from leadership.
